The Bravo box floats <br />and chains in the 1/2 (91) UDC sump and the 3/4 (89) UDC sump failed to stop the flow of product at the dispenser <br />when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br />product at the dispenser when it detects a leak. The service technician adjusted the floats and chains and verified <br />functionality. This is a repeat violation, Class II. <br />310 <br />Sensor in a piping sump failed to activate an audible and visual alarm or stop the flow product. The 87 STP sump <br />sensor set off a sensor out alarm when removed from the sump, and failed to activate an audible and visual alarm when <br />tested to ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an <br />audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor was replaced <br />and retested during the inspection. This is a minor violation. <br />319 <br />Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). Liquid was observed <br />in the 89 fill sump, 87 STP sump, 89 STP sump, and 91 STP sump. If water could enter into the secondary containment <br />by precipitation or infiltration, it must be removed and disposed of properly. The service technician removed the liquid <br />from the sumps. Ensure that all sumps and annular spaces are maintained free of liquid. This is a repeat violation, <br />Class II. <br />323 <br />Leak detector failed to detect a 3.0 gph leak at 10 psi. The 89 and 91 VMI line leak detectors failed to detect a leak <br />when tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service <br />technician adjusted the leak detectors, retested them, and they passed. This is a repeat violation, Class II. <br />326 <br />Overfill prevention system was overridden or failed to meet overfill requirements. The mechanical overfill prevention <br />device on the 91 UST is currently overridden by the presence of a screwdriver placed in the drop tube at the flapper. <br />The screwdriver was removed from the flapper by the service technician on site. Take all necessary precautions to <br />prevent this in the future. This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by September 19, 2014. <br />Consent to perform the inspection, take photos and collect samples was given by Pargat Litt, Franchisee. This <br />facility's designated operator is Curtis Carpenter (ICC expiration date: 3/20/15) . The service technician was <br />Jessie Wadkins (ICC expiration date: 4/29/15), who had the following manufacturer's certifications: VR x. 5/4/15, <br />VMI x. 2/6/16. <br />Please be aware that facility operators are required to upload the following UST program documents into the <br />California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial <br />Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if applicable), UST Letter from <br />Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance.
