Immediately log into the California Environmental Reporting System(CERS)at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a repeat violation, Class II. <br /> 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 8/19/2017 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by 9/18/2017. This <br /> information was received on 10/26/17 resulting in a non-compliant status for this facility. An operator that receives <br /> an inspection report shall have 30 days to submit a written response that includes a statement documenting <br /> corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response <br /> documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. <br /> This is a repeat violation, Class II. <br /> 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br /> Current financial responsibility documents were not found on site. These records shall be maintained on site for at <br /> least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring <br /> equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis <br /> performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment,ground water <br /> elevations, or other test results; (6)results of inventory readings and reconciliations. Immediately locate and <br /> maintain all maintenance and monitoring records for the last three years on site. <br /> This is a minor violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to check and note the correct expiration date for the designated operator expiration <br /> date.Two different expiration dates were noted in the monthly reports. The October 18th, 2017, November 6th, <br /> 2017, December 4th, 2017, January 10th,2018 state that the expiration date for the designated operator, Harold <br /> Largo,to be November 4th,2018. However the September 6th,2017, and the February 13th, 2018 state that the <br /> expiration date for the designated operator, Harold Largo,to be November 13th,2018. During the monthly <br /> inspection, the designated operator shall check that all required testing and maintenance for the UST system have <br /> been completed and document the dates they were done.
