Furthermore the hire date for some of <br /> the employees were stated as"refresh"and not an actual date. Ensure that actual dates are stated in the employee <br /> training log. The designated operator shall train facility employees for which he or she is responsible in the proper <br /> operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited <br /> to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br /> The 87 product STP sump sensor failed to activate an audible and visual alarm when tested to ensure continuous <br /> monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm. <br /> The sensor was adjusted and retested with passing results during the inspection . <br /> This was corrected on site <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 87 product, 89 product, and 91 product STP sumps. approximately 20 gallons of liquid <br /> was also observed in the 87 product fill sump, approximately 100 gallons of liquid was observed in the 89 product <br /> fill sump, and approximately 90 gallons of liquid was observed in the 91 product fill sump. If water could enter into <br /> the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Ensure that <br /> the 87 product, 89 product, and 91 product STP sumps and the 87 product, 89 product, and 91 product fill sumps <br /> are maintained free of liquid. <br /> The service technician removed the water from the 87 product,89 product, and 91 product STP sumps. However <br /> the liquid in the 87 product,89 product, and 91 product fill sumps was not removed. Immediately remove this <br /> liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. <br /> This is a repeat violation, Class II. <br /> 318 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 89 product and 91 product VMI line leak detector failed to detect a leak when tested. All line leak detectors <br /> shall be capable of detecting a 3-gallon per hour leak at 10 psi.
