A new designated operator a started the monthly visual inspection report from October, <br /> November, December 2011 and January 2012 and notification was not provided to the EHD within 30 <br /> days of the change. An Owner Statement of new DO was submitted to EHD in July 2011 for the new <br /> DO for this facility but only listed Service Station System's Dave Thomas and Shane Flores as facility's <br /> DOs. Any changes shall be reported to the EHD with 30 days. Submit notification to the EHD identifying <br /> all the designated operators for this facility. <br /> 204. The designated operator failed to visually inspect the the UST system for leaks, water, or debris <br /> during the monthly inspections of March, April and May 2011. During the monthly inspection, the <br /> designated operator shall visually inspect all spill containers and UDC sumps, and any tank top sump <br /> that had an alarm for which there is no record of a service visit. Ensure that designated operators <br /> performing monthly inspections at this facility are inspecting all of the required areas and noting it on the <br /> on the reports. <br /> 205. The March, April and May 2011 designated operator monthly inspection reports were not found on <br /> site. DO report for February 2011 was performed by Epic Compliance Brian Dunahay. The DO reports <br /> resumed in June 2011 by Service Station System Dave Thomas. Designated operator monthly <br /> inspection reports for the previous twelve months shall be retained on site. Locate and ensure that <br /> copies of the previous twelve months of designated operator monthly inspection reports are maintained <br /> on site.
