Since it appears that some underground polishing lines will be removed, fill out <br />Page 5 (Sampling Protocol for Routine Piping/Dispenser Removal/Replacement) of the <br />Retrofit or Piping Repair Permit application, which was submitted blank, and submit for <br />review to the EHD. <br />5. Page 6 of the Retrofit or Piping Repair Permit, the authorization to "Release Analytical <br />Results", which was provided as part of this permit application, lacks the Analytical <br />Laboratory information, please revise and submit to the EHD for review. <br />6. Please explain how the existing day tank will be decontaminated and disposed of, and <br />how will the rinse water be handled. <br />7.
