Zach, <br />CA478419 / Manteca CO was written up for the following: <br />104. Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. <br />A copy of current financial responsibility documents was provided to the EHD at the time of inspection. CORRECTED ONSITE. <br />302 & 308. The test boots were not pulled back and Schrader valves were not removed in the fill sump and in the transition sump. Any potential release from the <br />primary containment into the secondary containment in the diesel piping does not flow into a collection sump. All secondary containment systems shall be <br />sloped so that all releases will flow to a collection sump located at the low point of the underground piping. During the inspection the technician removed the <br />Schrader valve in the transition sump and pulled back the boot and removed the Schrader valve on the fill sump piping. CORRECTED ONSITE. <br />109. Annual monitoring system certification, leak detector testing, and spill container testing was performed on August 24, 2012 and a test report has not been <br />submitted to the EHD within 30 days of testing. A copy of the test report must be submitted within 30 days of the tests. Ensure that future reports are submitted <br />to the EHD within the required time frame. — Corrected - San Joaquin County Environmental Health had just moved when the 2012 report was mailed September <br />20, 2012. The correct address has been updated in our system. <br />204. The designated operator failed to check and note the correct test dates for the monitoring system certification and secondary containment testing on the <br />following designated operator monthly inspection reports: September 2012, October 2012, November 2012, and December 2012. Furthermore, the November <br />2012 designated operator inspection report listed the inspection date as 11-0-2012. During the monthly inspection, the designated operator shall check that all <br />required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators <br />performing future monthly inspections at this facility are including all of the required information on the reports. <br />It appears that the DO out the incorrect date for the monitoring system certification for the September 2012 report. The DO perpetuated the mistake for <br />October 2012, November 2012 and December 2012 by noting the monitoring system certification date on the September 2012 DO report and failed to <br />look at the actual monitoring system certification date. — This matter will be discussed at length with staff! <br />Note: A copy of Owner Statement of Designated UST Operator form featuring updated ICC expiration dates was provided to the EHD at the time of inspection. <br />Thank you, <br />Michael Barber <br />General Manager <br />
