No person may own or operate an UST unless a permit for its operation has been issued by the local agency to the owner or operator of the UST system (HSC 25284). 2. Failure to comply with UST operating permit conditions (CCR 2712). 3. The diesel fill direct bury spill container failed when tested (CCR 2635 (b). 4. Failure to construct, operate, and maintain primary containment as product tight (HSC 25290.1, 25290.2, 25291). 5. Failed to have an approved UST monitoring plan by EHD and not found on site, (CCR 2632(d)(1), CCR 2712(1)). 6. Failed to have an approved UST response plan available onsite (CCR 2712(i), 2634(e)). 7. Failed to submit monitoring system certification test report within 30 days for 2017, 2013 (CCR 2638 (d)). 8. Failed to submit a written response within 30 days of receiving an inspection report (CCR 2712 (f)). 9. Failed to maintain alarm logs and/or follow up actions for the past three years (CCR 2712(b)).
