The following is an itemized list of underground storage tank violations that have not <br />been addressed for QUICK SHOP as of June 29, 2017. <br />Open violations from March 31, 2017 inspection <br />Violation #101 - No permit to operate. <br />Fees for the annual permit to operate the UST system have not been paid, in addition the Board of Equalization <br />(BOE) number 44037432 submitted into CERS is not a valid number. No person may own or operate an UST <br />unless a permit for its operation has been issued by the local agency to the owner or operator of the UST system. <br />Immediately contact the EHD accounting office to make arrangements for payment to obtain a permit to operate a <br />UST system and correct BOE number in CERS. <br />Violation #103 - Current financial responsibility documents not submitted. <br />Financial responsibility documents have not been submitted to the EHD. The most recent CERS submittal for the <br />financial responsibility documents is dated as 03/19/2016 and the CFO letter is dated 03/10/2016. Current financial <br />responsibility documents are required to be submitted annually. Immediately log into the California Environmental <br />Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. <br />Violation #105 - Failed to have an approved UST monitoring plan. <br />The monitoring plan is not current and/or not approved by the EHD. The annular sensor for the 87 product is listed <br />as model 470, which appears to be incorrect per technician. The monitoring plan must be uploaded to the California <br />Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit <br />for review by the EHD. <br />Violation #107 - Plot plan/site map not submitted or failed to completely show where monitoring is <br />performed. <br />An accurate UST Monitoring Site Plan was not submitted. The UST Monitoring Site Plan does not include the liquid <br />sensor location for the 87 product and the 100 octane product liquid sensors located in the Submersible Turbine <br />Pump (STP) sumps. A site plan must be submitted identifying the locations where monitoring will be performed. <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload <br />a copy of the UST Monitoring Site Plan. <br />Violation #111- Failed to test monitoring and leak detection equipment annually. <br />An annual monitoring system certification (MSC) and leak detector testing were performed on 9/23/2013 and on <br />2/27/2015, and on 3/22/2016. The annual monitoring system certification was due by 9/23/2016 per the 03/22/2016 <br />inspection. Testing has not been completed and is 6 months past due. The 100 and 110- octane monitoring <br />systems were not tested today. These tests are required once every 12 months. THe next routine UST inspection <br />and monitoring system certification is due by 9-23-2017. Take all necessary precautions to ensure testing is <br />performed in a timely manner. <br />Violation #112 - Failed to test spill containers annually. <br />An annual spill container certification was performed on 9/23/2013 and on 2/27/2015, and on 3/22/2016. The <br />annual spill container certification was due by 9/23/2016 per the 03/22/2016 inspection. Testing has not been <br />completed and is 6 months past due. The 100 and 110- octane spill containers were not tested today. These tests <br />are required once every 12 months. THe next routine UST inspection and spill container certification is due by <br />9-23-2017. Take all necessary precautions to ensure testing is performed in a timely manner. <br />Violation #113 - Failed to submit monitoring system certification test report within 30 days. <br />A monitoring system certification, leak detector testing, and spill container testing were performed on 03/22/2016. A <br />test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). A copy <br />of the test report must be submitted within 30 days of the test.
