E <br />The following is an itemized list of underground storage tank violations that have not <br />been addressed for QUICK SHOP as of June 29, 2017. <br />Open violations from March 31, 2017 inspection <br />Violation #121 - Failed to submit a written response within 30 days of receiving an inspection report. <br />An inspection was last done on March 22nd 2016 and an inspection report was issued identifying information to be <br />submitted to bring this site into compliance. This information was required to be submitted by April 21 st 2016. <br />Several violations are still outstanding resulting in a non-compliant status for this facility. An operator that receives <br />an inspection report shall have 30 days to submit a written response that includes a statement documenting <br />corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response <br />documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. <br />Violation #204 - Current monitoring plan approved by the EHD not found on site. <br />An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br />accessible on site at all times. Immediately locate and retain a copy on site. <br />Violation #205 - Failure to have an approved UST response plan available onsite. <br />An approved copy of the response plan plans were not available for inspection. A copy of these plans shall be <br />accessible on site at all times. Immediately locate and retain a copy on site. <br />Violation #328 - Failed to comply with temporary closure requirements. <br />The 100 and 110 -octane UST system was found to be illegally abandoned. This violation was first noted on the" <br />Notice to Abate Immediately' issued by the EHD on 4/13/2013. A permit to repair the failed 100-ocatane spill <br />container was obtained on 3/06/2015, no spill container repairs conducted at this time. Furthermore, the 100 -octane <br />turbine has disconnected power source. No person shall abandon an UST system without complying with the <br />requirements of HSC 25298. Immediately obtain a permit from the EHD to temporarily close this UST system. A <br />permit application was submitted to the EHD for the temporary closure, however since the permit application was <br />not complete it was not approved. Submit all the required information for the above permit application and <br />temporarily close this UST system, as required. <br />Open violations from June 10, 2016 inspection <br />Violation #111- Failed to test monitoring and leak detection equipment annually. <br />Annual monitoring system certification and leak detector testing were last performed on 9-23-2013 and on <br />2-27-2015, and was completed on 3-22-2016. Also, the diesel leak detector was not tested until 6-10-2016, and is 9 <br />month past due. These tests are required once every 12 months. The next routine UST inspection and monitoring <br />system certification are due by 9-23-2016. Take all necessary precautions to ensure testing is performed in a timely <br />manner. <br />Violation #318 - Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 100 -octane leak detector failed on 3-22-2016 and the 110 -octane line leak detector could not be tested on <br />3-22-2016 due to a disconnected power source. All line leak detectors shall be capable of detecting a 3 -gallon per <br />hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor repair or replace the <br />failed leak detector (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the <br />failed leak detector can't be replaced immediately, this UST system will remain red tagged to prevent fuel inputs. <br />Violation #319 - No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br />The 100 -octane spill container was not tested on 3-22-2016, the technician indicated that it failed during last testing <br />and no repairs were made since the last test. All USTs shall be equipped with a spill container that has a minimum <br />capacity of five gallons and a drain valve that allows drainage of the collected spill into the primary container or <br />provide a means to keep the container empty. Immediately contact a properly licensed, trained, and certified <br />contractor to install an approved spill container under permit and inspection of the EHD. If a spill container can't be <br />installed immediately, this system will remain red tagged to prevent fuel inputs. <br />Page 2 of 5 <br />
