C <br />n <br />The following is an itemized list of underground storage tank violations that have not <br />been addressed for QUICK SHOP as of March 31, 2017. <br />Open violations from June 10, 2016 inspection <br />Violation #111- Failed to test monitoring and leak detection equipment annually. <br />Annual monitoring system certification and leak detector testing were last performed on 9-23-2013 and on 2-27-2015 <br />and was completed on 3-22-2016. Also, the diesel leak detector was not tested until 6-10-2016, and is 9 month past <br />due. These tests are required once every 12 months. The next routine UST inspection and monitoring system <br />certification are due by 9-23-2016. Take all necessary precautions to ensure testing is performed in a timely manner. <br />Violation #318 - Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 100 -octane leak detector failed on 3-22-2016 and the 110 -octane line leak detector could not be tested on <br />3-22-2016 due to a disconnected power source. All line leak detectors shall be capable of detecting a 3 -gallon per <br />hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor repair or replace the <br />failed leak detector (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the <br />failed leak detector can't be replaced immediately, this UST system will remain red tagged to prevent fuel inputs. <br />Violation #319 - No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br />The 100 -octane spill container was not tested on 3-22-2016, the technician indicated that it failed during last testing <br />and no repairs were made since the last test. All USTs shall be equipped with a spill container that has a minimum <br />capacity of five gallons and a drain valve that allows drainage of the collected spill into the primary container or <br />provide a means to keep the container empty. Immediately contact a properly licensed, trained, and certified <br />contractor to install an approved spill container under permit and inspection of the EHD. If a spill container can't be <br />installed immediately, this system will remain red tagged to prevent fuel inputs. <br />Violation #328 - Failed to comply with temporary closure requirements. <br />The 100 and 110 -octane UST system was found to be illegally abandoned. This violation was fist noted on the Notice <br />to Abate Immediately issued by the EHD on 4-13-2013. A permit to repair the failed 100 -octane spill container was <br />obtained on 3-6-2015, no spill container repairs were conducted at this time. Furthermore, the 100 -octane turbine <br />has a disconnected power source. Mr. Chahoud stated that the entire concrete slab over the split 100, 110, and <br />diesel split tank is sinking due heavy vehicular traffic in that area. No person shall abandon an UST system without <br />complying with the requirements of HSC 25298. Mr. Chahoud , tank owner, states the UST system will be reused bu. <br />he has no funds to repair it at this time. Immediately obtain a permit from the EHD to temporarily close this UST <br />system. <br />This is a repeat violation, Class II. <br />Open violations from March 28, 2016 inspection <br />Violation #318 - Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 100 -octane line leak detector failed on 3-22-2016, the 110 -octane line leak detector couldn't be tested on <br />3-22-2016 due to a disconnected power source, and the diesel line leak detector was in slow flow prior to testing and <br />could not be tested at the time of inspection. All line leak detectors shall be capable of detecting a 3 -gallon per hour <br />leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed <br />leak detector (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If these <br />failed leak detectors can't be replaced immediately, this UST system will remain red tagged to prevent fuel inputs. <br />Submit proof of correction to the EHD. <br />Page 1 of 4 <br />
