The following is an itemized list of underground storage tank violations that have not <br />been addressed for QUICK SHOP as of June 29, 2017. <br />Open violations from February 20, 2015 inspection <br />Violation #121 - Failed to submit a written response within 30 days of receiving an inspection report. <br />An inspection was last done on September 23, 2013 and an inspection report was issued identifying information to <br />be submitted to bring this site into compliance. This information was required to be submitted by October 23, 2014. <br />This information has not been received resulting in a non-compliant status for this facility. Multiple follow up letters <br />were sent to this facility detailing the outstanding violations and the required corrective actions. No return to <br />compliance documentation was received for May 31, 2011, September 28, 2011, and September 28, 2013 routine <br />UST inspections. Immediately complete and submit copies of respective return to compliance documentation with <br />statement of how each historical violation was corrected, including supporting documentation. <br />An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br />statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a <br />written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br />inspection report. Immediately provide complete and accurate return to compliance documentation for all the <br />aforementioned inspections. This is a Class II violation. <br />Violation #201 - Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance and monitoring records for the last three years were not found on site. These records shall be <br />maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or <br />sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) <br />results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring <br />equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. <br />Several alarms were noted during this inspection, as follows: L13 (2-21-2-14), L12 (2-21-2014), L11 (2-21-2014, <br />2-26-2014), L10 (2-21-2014, 2-25-2014, 2-26-2014, 9-25-2014,10-31-2014), and L6 (11-14-2014). Immediately <br />locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the <br />EHD. This is a Class II violation. <br />Violation #325 - Spill container failed to contain a minimum capacity of five gallons. <br />The 100 -octane direct -bury spill container failed when tested during the last monitoring system certification <br />(September 23, 2013). According to the fuel inventory obtained today, the 100 -octane tank has 918 gallons and <br />110 -octane tank has 603 gallons of fuel. According to Mr. Chahoud, fuel was deposited into this tank and the tank is <br />being actively used. According to the last inspection report, during which this spill container has failed, this tank did <br />not have enough fuel to perform the leak detector testing. As such, the fuel was deposited into the failed spill <br />container since the last monitoring system certification, which occurred on September 23, 2013. <br />All spill containers shall have a minimum capacity of five gallons and be capable of containing a spill or overfill until it <br />is detected or cleaned up. Immediately discontinue deposition of petroleum into this tank until the component is <br />repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br />IF THE SPILL CONTAINER CAN'T BE REPLACED IMMEDIATELY, THERE IS A POSSIBILITY THAT THE <br />100 -OCTANE UST SYSTEM MAY BE RED TAGGED TO PREVENT FUEL INPUTS This is a repeat violation, <br />Class IL <br />Open violations from September 23, 2013 inspection <br />Page 5 of 5 <br />
