The following is an itemized list of underground storage tank violations that have not <br />been addressed for QUICK SHOP as of June 29, 2017. <br />Open violations from March 22, 2016 inspection <br />Violation #318 - Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 100 -octane line leak detector failed, the 110 -octane line leak detector couldn't be tested due to a disconnected <br />power source, and the diesel line leak detector was in slow flow prior to testing and could not be tested at the time of <br />inspection. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have <br />a properly licensed, trained, and certified contractor repair or replace the failed leak detector (LG 113-x listed, if <br />applicable) and obtain a permit within one business day from the EHD. If these failed leak detectors can't be <br />replaced immediately, this UST system will remain red tagged to prevent fuel inputs. Submit proof of correction to <br />the EHD. <br />Violation #325 - Spill container failed to contain a minimum capacity of five gallons. <br />The 100 -octane spill container was not tested, the technician indicated that it failed last time and no repairs were <br />made since the last test, the diesel spill container direct -bury spill container failed when tested. All spill containers <br />shall have a minimum capacity of five gallons and be capable of containing a spill or overfill until it is detected or <br />cleaned up. Immediately discontinue deposition of petroleum into this tank until the component is repaired or <br />replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. If these <br />spill containers can't be replaced immediately, this UST system will remain red tagged to prevent fuel inputs. <br />Violation #328 - Failed to comply with temporary closure requirements. <br />The 100 and 110 -octane UST system was found to be illegally abandoned. This violation was fist noted on the <br />Notice to Abate Immediately issued by the EHD on 4-13-2013. A permit to repair the failed 100 -octane spill <br />container was obtained on 3-6-2015, no spill container repairs were conducted at this time. Furthermore, the <br />100 -octane turbine has a disconnected power source. Mr. Chahoud stated that the entire concrete slab over the <br />split 100, 110, and diesel split tank is sinking due heavy vehicular traffic in that area. No person shall abandon an <br />UST system without complying with the requirements of HSC 25298. Mr. Chahoud , tank owner, states the UST <br />system will be reused but he has no funds to repair it at this time. Immediately obtain a permit from the EHD to <br />temporarily close this UST system. A permit application was submitted to the EHD for the temporary closure, <br />however since the permit application was not complete it was not approved at this time, submit all the required <br />information for the above permit application and temporarily close this UST system, as required. <br />Open violations from March 27, 2015 inspection <br />Violation #325 - Spill container failed to contain a minimum capacity of five gallons. <br />The 100 -octane direct -bury spill container failed when tested. All spill containers shall have a minimum capacity of <br />five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately discontinue <br />deposition of petroleum into this tank until the component is repaired or replaced by a properly licensed, trained, and <br />certified contractor under permit and inspection of the EHD. If the spill container can't be replaced immediately, <br />there is a possibility that the 100 -octane UST system may be red tagged to prevent fuel inputs. Product level in the <br />100 -octane tank was reported as 877 -gallons. <br />Open violations from February 20, 2015 inspection <br />Violation #113 - Failed to submit monitoring system certification test report within 30 days. <br />A monitoring system certification, leak detector testing, and spill container testing were performed on September 23, <br />2013 and a test report has not been submitted to the the EHD or to the California Environmental Reporting System <br />(CERS). A copy of the test report must be submitted within 30 days of the test. Immediately provide a copy of <br />above test report to the EHD or log into CERS at http://cers.calepa.ca.gov and upload a copy of the test report as a <br />miscellaneous State -Required Document in the Underground Storage Tank section This is a repeat violation, <br />Class II. <br />Page 4 of 5 <br />
