Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br />Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br />maintained on site. Submit copies to the EHD. <br />This is a repeat violation, Class II. <br />329 <br />HSC 25298 Failed to comply with all permanent closure requirements. <br />This UST system was found to be illegally abandoned. No person shall abandon an UST system without complying <br />with the requirements of HSC 25298. Immediately obtain a permit from the EHD to remove this UST system. A <br />"Failure to Obtain a Permit' penalty applies. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by November 18th, 2018. <br />Documents reviewed: Financial responsibility document, response plan, monitoring plan, monitoring system <br />certification test results for 2015, 2016, 2017, maintenance records, designated operator monthly inspection <br />reports, and designated operator training records. <br />Documents provided: Return to compliance certification. <br />Starting September 1, 2018, all in -office CERS help will be provided at EHD hourly rate ($152). To schedule an <br />appointment, please call (209) 468-3420. <br />NOTE: This routine inspection was for paperwork only. The monitoring system certification was canceled by <br />owner because the UST system did not have fuel for testing. The owner stated that the monitoring system <br />certification will be reschedule once the site has fuel for testing.
