An inspection was last done on 5/25/2017 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 6/24/2017 . This information was received on 7/17/2017 resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a Class II violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment test on the June 2017 through May 2018 designated operator monthly inspection report. The 12 inspection reports stated that secondary containment testing wa performed on 8/4/2016 however the test results that were found on site stated that the test was performed on 8/3/2016. Also on the March 12, 2018 DO monthly report the report states February 12th, 2018 however the alarm history states Marchl2, 2018. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 309 CCR 2636(f)(1) Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow product. The 87 grade Bravo box float and chain in Under Dispenser Containment (UDC) 3/4 secondary containment failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician clean out the float and verified functionality. This was corrected on site. This is a repeat violation, Class ll. 319 CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. The 91 grade UST spill container failed when tested. All USTs shall be equipped with a spill container that has a minimum capacity of five gallons and a drain valve that allows drainage of the collected spill into the primary container or provide a means to keep the container empty. The Service technician on site, Anthony, Reyes replaced the spill container with another spill container that was able to hold 5 gallons for at least 60 minutes under the inspection of the EHD inspector. This was corrected on site. This is a minor violation.
