This information was required to be submitted by 7/10/2016. This <br />information was received on 3/2/2017, approximately 8 months late. An operator that receives an inspection report <br />shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or <br />proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions <br />taken or proposed is submitted within 30 days of receiving an inspection report. <br />This is a repeat violation, Class II. <br />201 <br />CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br />On the 4/3/2017 inspection report, the DO noted "Diesel spill bucket in need of repair. Manager states service <br />placed and waiting on parts." The work order or documentation that the spill container had been repaired was not <br />found on site. Immediatly locate and submit this work order for the repair of the diesel spill container to the EHD. <br />Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br />maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br />field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; <br />(6) results of inventory readings and reconciliations. <br />This is a repeat violation, Class II. <br />209 <br />CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator (DO) failed to check and note the correct test dates. For the July 8, 2017 inspection <br />report, the DO noted the monitoring system certification and the spill container tests were performed on 6/10/2015. <br />The correct date is 6/10/2016. For September 1, 2016, October 13, 2016, and November 3, 2016 inspection <br />reports, the DO noted the secondary testing occurred on 8/8/2013. The correct date is 8/8/2016. During the <br />monthly inspection, the designated operator shall check that all required testing and maintenance for the UST <br />system have been completed and document the dates they were done. Ensure that designated operators <br />performing monthly inspections at this facility are including all of the required information on the reports. <br />This is a repeat violation, Class II. <br />211 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The designated operator employee training for Komal Chohan was not on the training log and the hire dates were <br />not listed for all of the employees on the training log. The designated operator shall train facility employees for which <br />he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The <br />training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4.
