Elite IV on 12/05/2017 replaced <br />the 87 product line leak detector and diesel product line leak detectors was replace on 10/16/17. Both of those <br />replacement were like for like and EHD was notified of the replacements however the site did not have records of <br />the replacement. These records shall be maintained on site for at least three years. Monitoring records include: (1) <br />date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) <br />results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all <br />readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for <br />the last three years on site and submit copies to the EHD. <br />This is a repeat violation, Class ll. <br />204 <br />CCR 2712(1) Current monitoring plan approved by the EHD not found on site. <br />An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br />accessible on site at all times. Immediately locate and retain a copy on site. <br />This is a minor violation. <br />211 <br />CCR 2715(f)(2) Designated operator employee training not performed or log not kept. <br />The designated operator employee training did not have the hire date for any of the employees trained. The <br />designated operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4.
