DeVries Road, Lodi, CA <br />Dear Mr. Cacapit: <br />On September 13, 2010 you completed an Underground Storage Tank Inspection at 13850 <br />DeVries Road in Lodi, CA. As part of the inspection, you listed the following items that <br />required immediate attention. Below is my response for each item: <br />Item #9: UST system is not operated to prevent unauthorized release, including spills and <br />overfills: Balch Petroleum has been contracted to identify and repair any leaks in the UST <br />system. A UST Repair Permit was issued by San Joaquin County on October 7, 2010. Repair <br />work will start on October 14 or 15, 2010. Once work is completed, additional information <br />will be provided to San Joaquin County. <br />Item #14: Primary containment is not product tight: Balch Petroleum has been contracted to <br />identify and repair any leaks in the UST system. A UST Repair Permit was issued by San <br />Joaquin County on October 7, 2010. Repair work will start on October 14 or 15, 2010. Once <br />work is completed, additional information will be provided to San Joaquin County. <br />Item #22: Unauthorized Release: A small amount, about half of cup of red dye diesel was at <br />the bottom of the piping/transition area was not recorded. On September 13, 2010, Jonathan <br />Pakele of MCI, doing business as, Verizon Business completed the "Recording Requirements <br />for Unauthorized Releases" form for the approximately one half cup of diesel fuel. Jonathan <br />also cleaned up the diesel fuel with an absorbent pad. <br />Item #23c: Designated Monthly Inspection: The UST designated monthly inspection for <br />August 2010 was not completed by Gary Duvall. Gary was the MCI, doing business as, <br />Verizon Business designated operator for this location. Due to a confidential internal company <br />human resources issue, Gary was placed on temporary leave in mid-August 2010. Since this <br />was a confidential internal human resources issue, Gary was only allowed to speak with his <br />manager and human resources.
