SUMMARY <br />On May 23, 2002, the EHD mailed a letter to Sutter Tracy Community Hospital notifying <br />them that they needed to perform an annual monitoring certification and line leak <br />detector test on their underground storage tank (UST) by June 24, 2002 (Attachment 1). <br />In addition, <br />this letter also reminded the facility to perform the secondary containment test by January <br />1, 2003. <br />On July 12, 2002, Michelle Le, Senior REHS, performed a routine inspection and <br />witnessed the monitoring certification. Ms. Le reminded the plant operations manager to <br />perform secondary containment testing on the UST interstitial space and sump by January <br />1, 2003 (Attachment 2). <br />On September 30, 2002, an EHD notification letter was mailed to the facility stating <br />secondary containment testing must be completed by January 1, 2003 (Attachment 3). In <br />addition, numerous CUPA newsletters were mailed to the facility stating the secondary <br />containment test deadline (Attachment 4). <br />On October 15, 2002, Champion Precision Tank Testing Company was on site (1420 <br />N. Tracy Blvd., Tracy) to perform a secondary containment test. On the secondary <br />containment test report, it indicated that the sump was not tested due to lack of a test boot <br />(Attachment 5). <br />On November 06, 2003, the hospital was mailed a letter stating that an annual monitoring <br />certification and line leak detector test must be performed on the UST by December 6, <br />2003 (Attachment 6). <br />On January 22, 2004,,,'Steven Shih, Senior REHS performed a routine UST and hazardous <br />waste inspection and witnessed the monitoring certification (Attachment 7).
