NAME: piping was changed. Ms. Sorensen did notknow if piping is underground or DBA: Sutter Tracy Community Hospital aboveground. I asked Ms. Sorensen to submit scope of work and cut sheet. Kieth Nichols, Plant Operations Manager 06/18/2004 Received fax copy of scope of work and cut sheets. The scope of work is to SCS 1420 N. Tracy Blvd., Tracy, CA 95376 upgrade fuel oil piping to boilers during generator replacement. I called Ms. (209) 832-6032 IV. INVESTIGATING OFFICERS (INCLUDING OTHER AGENCIES) Carolyn Sorensen and asked if she know the detail of work. Ms. Sorensen transferred me to Mr. Kieth Nichols. Mr. Nichols stated that the portion of underground piping was rerouted to the new generator. In addition, the existing steel piping was replaced with flex piping. Mr. Nichols also stated that the trench is still opened. I asked Mr. Nichols if there is permit to perform this work? Mr. Nichols did not aware EHD permit is required. I told Mr. Nichols that a permit is required to perform UST piping removal and replacement. An approved permit is required prior to commerce work, and then inspections will follow. Mr. Nichols stated that he will contact contractor AIRCO to get in touch with me for the permit requirements. 07/13/2004 I received phone call from Jay Thangaraj from DPR Construction regarding SCS permit application requirement. Mr. Thangaraj stated that he obtained permit application from web site. Mr. Thangaraj stated that the old piping will be replaced with new pipe in different trench. 07/15/2004 Filed Financial Responsibility SCS 08/11/2004 Received Permit Application SR0039176 from DPR Construction to reroute SCS piping to new backup generator. 08/18/2004 Issued deficient letter to DPR Construction. SCS 08/25/2004 Received second drawing, but still not satisfy the deficiency letter et. SCS 09/02/2004 Talked to Terry Hoshi (402) 399-1296 of HDR regarding deficiency letter. Mr. SCS Hoshi stated that HDR is design company while DPR is construction company. 09/07/2004 Received revised drawing, but still shows deficiency/inconsistency of types of SCS piping in the plan. 09/09/2004 Received revised drawing and approved the plan to retrofit the piping. See SCS permit approval letter dated on 09/09/2004 09/10/2004 Filed Monitoring and Response Plan. SCS 09/15/2004 Witnessed the soil sampling. 2 samples taken from piping trench and 2 samples SCS taken from stockpile. Existing piping had already been removed and disposed to Altamont Landfill (Copy of Non -hazardous waste manifest received on site). 09/20/2004 Witnessed the primary piping pressure and soap test. Passed the test and start SCS the lake test. 09/21/2004 Witnessed the lake test passed. SCS 09/23/2004 Witnessed the secondary piping ressure and soap test passed. 09/28/2004 Slope check OK. SCS 10/07/2004 Final inspection conducted. This facility is OK to operate. Filed passed line SCS and tank test, stockpile disposal information and as -build.
