Beneath Record keeping, change"no"to"yes"for Alarm Logs and change"no"to"yes" for <br /> Visual Inspection Records. <br /> This is a repeat violation, Class II. <br /> 106 CCR 2632(d)(2) Failed to have an approved UST response plan. <br /> The response plan is not current and/or not approved by the Environmental Health Department (EHD). The <br /> response plan still lists Doug Archer as a responsible person even though Doug Archer is no longer at this facility. <br /> The response plan must be uploaded to California Environmental Reporting System (CERS). Immediately log into <br /> CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at <br /> any time. <br /> This is a repeat violation, Class ll. <br /> 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br /> An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations <br /> where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a repeat violation, Class ll. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Monitoring records for the last three years were not found on site. Monitoring system certification test results for <br /> 2013 were not found on site. These records shall be maintained on site for at least three years.
