The designated <br /> operator checked "yes"for"Each alarm for the previous month has been responded to appropriately"when there <br /> was no alarm during the previous month. This was corrected on site. There was no panel print out of alarms for the <br /> June 15, 2016 designated operator monthly inspection report. During the monthly inspection, the designated <br /> operator shall review the alarm history for the previous month, check that each alarm was documented and <br /> responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 210 CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br /> The November 2015 and October 2016 designated operator monthly inspection reports were not found on site. <br /> Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Ensure <br /> that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 319 CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br /> The diesel spill container contained about a cup of liquid. All USTs shall be equipped with a spill container that has <br /> a minimum capacity of five gallons and a drain valve that allows drainage of the collected spill into the primary <br /> container or provide a means to keep the container empty. Continue to inspect the spill container and remove any <br /> liquid when present. <br /> This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by December 4, 2016. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
