An accurate UST Monitoring Site Plan was not submitted. Location of the monitoring system control panel is not indicated on the site plan. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a repeat violation, Class II. 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on 11/4/2016 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 12/3/2016. This information was received after the 30 days. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a repeat violation, Class Il. 209 CCR 2715(c)Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to attach alarm history for all designated operator monthly reports for the past 12 months. During the monthly inspection,the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 306 CCR 2630(d)Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. The diesel sump sensor was observed attached to sump wall across from product piping and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest and near product piping in he sump by the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity. This was corrected on site. This is a Class II violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 12/17/2017.
