Location of the monitoring system control panel is not <br /> indicated on the site plan. A site plan must be submitted identifying the locations where monitoring will be <br /> performed. Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a repeat violation, Class II. <br /> 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 11/4/2016 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by 12/3/2016. This <br /> information was received after the 30 days. An operator that receives an inspection report shall have 30 days to <br /> submit a written response that includes a statement documenting corrective actions taken or proposing corrective <br /> actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is <br /> submitted within 30 days of receiving an inspection report. <br /> This is a repeat violation, Class Il. <br /> 209 CCR 2715(c)Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to attach alarm history for all designated operator monthly reports for the past 12 <br /> months. During the monthly inspection,the designated operator shall review the alarm history for the previous <br /> month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br /> history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 306 CCR 2630(d)Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The diesel sump sensor was observed attached to sump wall across from product piping and not located to detect a <br /> leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br /> possible opportunity. The sensor was relocated to the lowest and near product piping in he sump by the service <br /> technician.
