CCR 2715(c): Designated operator monthly inspection failed to document alarms. <br />Designated Operator Alex Jabbari failed to note a Setup Data Warning alarm on L7: Disp 1-2 dated 05-27-09 <br />at 4:32 PM and a Sensor Out alarm on Ll: 87 STP sump dated 05-27-09 at 12:47 PM. Designated UST <br />operator shall review the alarm history report or log for the previous month, and checking that each alarm <br />was not documented and responded to appropriately. A copy of the alarm history report or log, along with <br />documentation describing action taken in response to any alarm(s), shall be attached to the monthly visual <br />inspection report. Designated UST operator shall provide the owner or operator with a copy of each monthly <br />inspection report, and alert the owner or operator of any conditions discovered during the monthly visual <br />inspection that may require follow-up actions. Correct by submitting a revised Designated Operator's <br />monthly visual inspection report for the reported month with the alarms and the response action for each <br />alarm to SJC EHD within 30 days (01-24-09). Ensure all future alarms are documented with written <br />appropriate response to the alarms. <br />#24. CCR 2638(d): Monitoring equipment certification test report not submitted within 30 days. <br />Test results for the monitoring system certification conducted on 03-03-09 by Afforda-test have not been <br />received by SJC EHD. UST owners and operators shall submit a completed "Monitoring System <br />Certification" form to SJC EHD within 30 days after completion of the inspection. Correct by submitting <br />monitoring system certification test results conducted on 03-03-09 to SJC EHD within 30 days (01-24-09). <br />Complete and submit one copy of the Return to Compliance Certification (on the back of the <br />inspection report page), with a corrective action statement for violations # 7, 11, 21, 23(c)(1), and <br />24, along with the requested and any supporting documentation to SJC EHD within 30 days (01- <br />24-09) <br />Note: <br />Secondary Containment is due by 02/2010. <br />Financial responsibility: Certification of Financial Responsibility and Letter from Chief Financial Officer must <br />be submitted to SJC EHD (via in person, fax or email) by 01/2010. <br />Designated UST Operator annual employee training by Alex Jabbari is due by 01/2010. <br />Error on Tank set up: T1: Premium 91 and T2: Premium 91 — both labeled the same. Missing Label of <br />Regular 87.
