First aid kit and fire extinguisher will be located at the command <br />vehicle. The nearest telephone is located on site and the BC truck will also be equipped with a mobile <br />telephone. The emergency telephone numbers to be used to call for assistance are listed in the section on <br />Key Personnel and Responsibilities with the reference list of project contacts. <br />The implementation of the SSHP must be documented to assure employee participation and protection. <br />In addition, the regulatory requirements must be met for record keeping on training, medical surveillance, <br />injuries and illnesses, exposure monitoring, health risk information, and respirator fit -tests. <br />Documentation of each employee's activities is maintained by the HSO in Sacramento, California. <br />Documentation of the implementation of this plan will be accomplished using Attachments A through E, <br />which are forms for Site Activity and Briefing, Field Checklist for Implementation, Employee <br />Acknowledgement, Native of Unsafe Conditions, and Document of Environmental Monitoring Equipment, <br />respectively. The SSO is responsible for assuring completion of, or completing, all required <br />documentation. The monitoring for methane using the combustible gas meter will be recorded periodically <br />throughout all on-site work activities. SSO completes or assures completion of all attachments. <br />
