The permit has been approved <br /> with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Contact Garrett Backus to arrange the following inspection(s)(a 48 hour notice is <br /> required): Measurement of the drop tubes to verify the 95%tank volume shutoff. <br /> 3. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company),and <br /> b. A valid,current California UST Service Technician certificate issued by the <br /> International Code Council(ICC),and <br /> c. A valid,current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired,or serviced,and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 4. Pen-nit expires 180 days from date of approval. Note: If an extension to the expiration <br /> date is required,a written request must be submitted to this office prior to the expiration <br />
