The permit has been approved <br /> with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Contact Garrett Backus to arrange the following inspection(s)(a 48 hour notice is <br /> required): Measurement of the drop tubes to verify the 95%tank volume shutoff. <br /> 3. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council(ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired, or serviced,and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 4. Permit expires 180 days from date of approval. Note: If an extension to the expiration <br /> date is required, a written request must be submitted to this office prior to the expiration <br /> date. <br /> 5. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API, ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 6. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> Page 1 of 2 <br />
