The alarm logs did not document, <br />nor address, the Q1 Supreme and Q2 Regular PLLD Shutdown Alarms on 12/5/17 and 12/19/17, and all alarms on <br />1/18/18. The alarm logs documented, but did not address, the L7 Supreme Fill Fuel Alarms on 4/20/17 and 6/21/17, <br />Q1 Supreme and Q2 Regular PLLD Shutdown Alarms on 5131/17, and the Q2 Regular PLLD Shutdown Alarm on <br />6/1/17, and L8 Regular Fill Fuel Alarm on 7/21/17. <br />Also, a maintenance record for the alarms that occurred on 1/18/18 during sensor testing after the dispensers were <br />replaced, was not found on site. <br />These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of <br />all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br />observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br />monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br />reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br />site, and submit proof to the EHD showing that the above mentioned alarms were properly addressed. <br />This is a repeat violation, Class 11. <br />209 <br />CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to document all the alarms from the attached alarm history on the April and <br />December 2017 designated operator monthly inspection reports and failed to check that they were responded to <br />appropriately. The missing alarms include: L7 Supreme Fill Fuel Alarm on 3/21/17 and Q2 Regular PLLD Shutdown <br />Alarms on 1215/17 and 12/19/17. Also the alarm history printout attached to the June 2017 designated operator <br />monthly inspection report did not have the printout date. During the monthly inspection, the designated operator <br />shall review the alarm history for the previous month, check that each alarm was documented and responded to <br />appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the <br />monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of <br />the required information on the reports. <br />This is a repeat violation, Class 11. <br />315 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Liquid was observed in the following sumps: <br />- Premium fill sump (-1 cup of liquid) <br />- Premium submersible turbine pump (STP) sump (-1 cup of liquid) <br />- Regular Master STP sump (- 2.5 cups of liquid) <br />- Regular Slave STP sump (-2 cups of liquid) <br />If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed <br />of properly. Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste <br />regulations, and manage it accordingly.
