Hazelton Ave. <br />Stockton, CA 95205-6232 <br />Dear Vicki, <br />ENVIRONMENTAL HEALTH <br />PERMIT/SERVICES <br />Enclosed is a copy of the Return to Compliance Certification and UST Inspection Report <br />from your inspection at our Pacific Avenue Chevron located at 6633 Pacific Ave, Stockton, <br />CA on January 23, 2017. We have done the following to correct the violations noted: <br />• Violation #104: The UST Tank Information forms have been corrected as <br />requested via CERS. Copies attached. <br />• Violation #107: AffordaTest has corrected the UST Monitoring Site Plan as <br />requested and I have submitted the updated map via CERS. Copy attached. <br />• Violation #201: The Alarm Logs for January through May 2016 have been located <br />and filed in the environmental box as required. Copies attached. <br />• Violation 4209: I have spoken with our Designated Operator Michael Reimers and <br />he has reviewed this violation. He has v$,-,ured me that he will make an extra effort <br />to be more thorough and accurate on all fixture inspection reports. <br />Please let me know if there is anything else needed to clear these violations.
