On July 21, 2006, Mr. Ng performed a routine underground storage tank (UST) <br /> inspection (Attachment 4). Current financial responsibility was lacking; maintenance and <br /> monitoring records could not be located onsite; secondary repairs have not been made <br /> since failing testing on March 2, 2006; designated operator reports were being performed <br /> by an individual not listed on the designated operator statement; monitoring certification, <br /> leak detector and spill bucket testing are past due. Financial responsibility and a new <br /> owner statement of the designated UST operator were submitted during the time of <br /> inspection. <br /> On August 2, 2006,permit SR0046358 to replace the monitoring system and repair the <br /> failed UDCs was approved and faxed to Mr. Matas(Attachment 5). <br /> On August 15, 2006,Mr. von Flue witnessed the testing of the monitoring system and <br /> repaired UDCs. The monitoring system sensors all passed, but positive shutdown could <br /> not be verified since there was no fuel in the tanks. UDCs 9/10, 11/12 failed and 13S/14 <br /> was not tested(Attachment 6). <br /> On August 25, 2006,Mr. Ng witnessed the testing of positive shutdown for the gasoline <br /> dispensers and sumps. Mr. Sandhu was approved to operate the UST system to sell gas. <br /> The diesel side was still under repair. Mr. Sandhu was reminded that he is still in <br /> violation for failing to perform leak detector and spill bucket testing(Attachment 7). <br /> On August 30, 2006, Mr. Ng witnessed the lake test for UDCs 9/10, 11/12, 13S/14, and <br /> 87, 91, and diesel spill buckets. All passed an accelerated lake test. Diesel north spill <br /> bucket could not be tested due to a broken drain valve(Attachment 8). <br /> On September 1, 2006,Mr. Ng witnessed the testing of positive shutdown for the diesel <br /> dispensers and sumps. Also witnessed the lake test for the diesel north spill bucket, <br /> which passed. Mr. Sandhu was approved to operate the UST system to sell diesel and <br /> was reminded that leak detector testing is still past due and to schedule these tests <br /> immediately(Attachment 9). <br /> On September 25, 2006, leak detectors were scheduled to be tested by CPT. When Mr. <br /> Ng arrived onsite to witness this testing, Donlee Pump and San Joaquin Valley Air <br /> Pollution were already onsite getting ready to perform air tests. Since these tests require <br /> the turbines to be stopped, leak detector testing could not be performed today. CPT will <br /> call to reschedule. <br /> Attachment 10 is the report submitted by D&S Dragline Service, Inc. for the completed <br /> repairs on permit SR0046358. <br /> Attachments 11, 12 and 13 include parcel information, facility and owner information, <br /> and the facility file narrative, respectively. <br /> 2 <br />
