Telephone calls were placed to Dialysis North in <br />January 2007, requesting they respond to perform the contracted service. However, the company <br />refused, stating it was their intention to respond in February 2007. Consequently, Dialysis North <br />arrived in Stockton on February 6, 2007, to perform the test. Although, you were here that same <br />day for your inspection, the required documentation was not made available to you. Please find <br />ATTACHMENT #A (Secondary Containment Testing Report Form), to satisfy the violation <br />noted in Item #2 of your report. <br />Item #6 — "Monitoring & response plan is onsite" <br />Please find ATTACHMENT #B (Emergency Action Plan for Stockton CHP) and (Updated site <br />map and San Joaquin County Hazardous Materials Management Plan for the Stockton CHP <br />office) to satisfy the violation noted in Item #6. The site map and Hazardous Materials <br />Management Plan have also been updated in the San Joaquin County Environmental Health <br />Department website. <br />Item #7 — "Maintenance & monitoring records" <br />The Stockton CHP maintains a log for purposes of recording trouble and alarms with our <br />underground storage tank. Please find ATTACHMENT #C (Daily Repair Log for 2007) to <br />satisfy the violation noted in Item #7. Please advise if this is not sufficient. <br />Safety, Service, and Security <br />
