The current unified program consolidated forms (facility and tank forms) must be on file <br /> with this office. Correct immediately. Complete and submit the provided facility and tank forms to this <br /> office by 6-17-10. <br /> #4 The current certification of financial responsibility and CFO letter for 2009 and 2010 have not yet <br /> been submitted to this office. The current certification of financial responsibility and CFO letter must be <br /> on file with this office. Correct immediately. Complete and submit the provided certification of financial <br /> responsibility and CFO letter forms to this office by 6-17-10. <br /> #5 The current monitoring and response plans have not yet been submitted to this office. The current <br /> monitoring and response plans must be on file with this office. Correct immediately. Complete and <br /> submit the provided monitoring and response plan forms to this office by 6-17-10. <br /> #11 1 observed that the VR208 sensors in the 87 and 91 stp sumps were positioned at the highest point <br /> of the sump at the top of the saddle. In the D.O. report the D.O. also indicated that the sensor was <br /> improperly located. UST system monitoring equipment must be properly located to detect a leak at the <br /> earliest opportunity. Technician corrected on site by placing the sensors at the lowest point of the sump. <br /> #16 1 observed that the test boots in the 87 and 91 stp sumps did pQt'have a Schrader valve and were <br /> left clamped on the product piping. This prevents any release into the secondary pipe from reaching the <br /> 87 and 91 stp sumps and detection by the VR208 sensors in those sumps. All releases in secondary <br /> piping must be able to flow to a collection sump. Correct immediately. <br /> #18 The direct bury diesel spill container failed the 1 hour hydrostatic lake test. Immediately discontinue <br /> the deposition of fuel into this tank until the direct bury diesel spill container has been repaired/replaced <br /> by a licensed, trained, and certified contractor under permit by San Joaquin County Environmental <br /> Health Department. Correct immediately. <br /> #21 1 observed about 1 cup of liquid in the UDC1/2 and about 5 gallons of liquid each in the 87 and 91 <br /> stp sumps. Water in the secondary containment must be analyzed and properly disposed of. Technician <br /> corrected on site by pumping out the liquid and placing it into the hazardous waste drums on site. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> EHD Inspector: Aris Cacapit ems—' Receiv y: Title: <br />
