Instructions for Form Completion — One (1) Form Per Tank System <br />1. Fill in the owner information. <br />a. Each UST will have a specific number as administered by the state CUPAs. <br />b. Record the tank volume with the associated tank number. The volume should be <br />given in gallons unless the owner operator explicitly notes otherwise. <br />c. The blend percentage of biodiesel must be listed. The blend of biodiesel cannot <br />exceed 20 percent by volume. List the blend percentage in conventional language; <br />e.g. B20, B 10, etc. <br />2. List the manufacturer name and model associated with any of the corresponding Leak <br />Detection Equipment. Leak detection methods or equipment must be compatible for <br />biodiesel blends stored or intended to be stored, A statement from the leak detection <br />manufacturer is needed to testify that the leak detection method or equipment is compatible <br />for each UST to store biodiesel blends. Put a check in last column if the tank under variance <br />has a statement from the leak detection manufacturer. <br />3. The documentation from the leak detection manufacturer(s) must be attached with this form. <br />Each tank system must have a separate form. Please repeat the above instructions for <br />additional tanks. <br />4. Additional room is given in the Table to include any components not listed. <br />
