The monitoring plan is not current and/or not approved by the EHD. Beneath Tank Monitoring is Performed Using the Following Method(s),change Leak Sensor Model#to LS-01. Beneath Pipe Monitoring is Performed Using the Following Method(s),change Leak Sensor Model from LA-51 to LS-01;enter"Yes"for Leak Alarm Triggers Automatic Pump Shutdown,and"Yes"for Failure/Disconnect Triggers Pump Shutdown. Beneath Record Keeping, change"No"to"Yes"for Equipment Maintenance and Calibration Records. The monitoring plan must be uploaded to the California Environmental Reporting System(CERS). Immediately log into CERS, make the necessary changes,and submit for review by the EHD. This is a minor violation. 106 CCR 2632(d)(2)Failed to have an approved UST response plan. The response plan is not current and/or not approved by the EHD. The response plan must be uploaded to the California Environmental Reporting System(CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 113 CCR 2638(d)Failed to submit monitoring system certification test report within 30 days. A monitoring system certification, leak detector testing,and spill container testing were performed on December 18, 2015. A test report has not been submitted to the EHD or the California Environmental Reporting System(CERS). A copy of the test report must be submitted within 30 days of the test. Immediately provide this report to the EHD. This is a minor violation. 201 CCR 2712(b)Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. The Monitoring System Certification report for December 6,2013 testing was not found on site during the inspection. These records shall be maintained on site for at least three years. Monitoring records include:(1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records;(3)results of any visual observations; (4) results of sample analysis performed a lab or in the field;(5)logs of all readings of gauges or other monitoring equipment,ground water elevations,or other test results;(6)results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last submit copies to the EHD. �5 This is a minor violation. 205 CCR 2712(i)Failure to have an approved UST response plan available onsite.
