"J" St. <br /> Tulare, CA 95274 <br /> Subject: Waterloo Liquor,2512 Waterloo, Stockton, CA (SR0058393) <br /> The San Joaquin County Environmental Health Department(EHD)has reviewed the <br /> application for permit to install and program a new Incon TS550 monitor panel to replace <br /> the existing monitoring control panel Incon TS 1000. The permit has been approved with <br /> the following conditions: <br /> 1. Contact the local fire department, local planning or building department and the <br /> San Joaquin Valley Air Pollution Control District for any applicable permits. <br /> 2. Contact Jeffrey Wong(468-0335)to arrange the following inspection. A 48-hour <br /> notice is required: <br /> a. The EHD must witness the functionality of the monitoring system to <br /> ensure that monitoring has been restored to meet the requirements of <br /> California Code of Regulations Title 23 and the Health & Safety <br /> Code. All monitoring system sensors must be tested. Please be aware <br /> that the facility cannot operate before the EHD inspection and approval of <br /> this Post-Cold Start Functionality Test. <br /> 3. A current printout of the alarm history must be created and available for <br /> EHD inspection before it is cleared by any part of this work plan such as a <br /> "cold start". <br /> 4. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD) for approval prior to commencing work.All new addenda <br /> supersedes the previous ones. <br /> 5. If concrete is broken over or near the existing underground storage tanks(UST), a <br /> tank secondary containment test must be performed on the UST in accordance to <br /> manufacturer's guidelines. Testing must be done after returning the facility to <br /> operating condition. Results must be submitted to EHD within 7 days of <br /> returning the facility to operating condition. <br /> 6. All components must be certified by an independent testing organization and <br /> labeled as required by the organization. (UL, ULC,ANSI,API,ASTM, etc. See <br /> Page 1 of 2 <br />
