A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested,repaired, or serviced; and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 8. An"Underground Storage Tank Certification of Installation/Modification" form <br /> (Form C) shall be submitted by the owner or the owners'agent [pursuant to <br /> (CCR)2635(d)] before this upgrade will be considered complete by EHD. <br /> 9. Permit expires 180 days from date of approval.
