Union St. <br />Stockton, CA 95201 <br />DIRECTOR <br />Donna Heran, REHS <br />PROGRAM COORDINATORS <br />Margaret Lagorio, REHS <br />Robert McClellon, REHS <br />Jeff Carruesco, RENS, RDI <br />Kasey Foley, REHS <br />(Hand Delivered September 30, 2009) <br />Subject: Waterloo Liquor, 2512 Waterloo, Stockton, CA (SR0058424) <br />The San Joaquin County Environmental Health Department (EHD) has reviewed the <br />application for permit to reprogram the existing Monitoring Control Panel Incon 1000. <br />The work has been performed on September 30, 2009 prior to EHD approval on <br />emergency repair basis. The permit has been approved with the following conditions: <br />1. Contact the local fire department, local planning or building department and the <br />San Joaquin Valley Air Pollution Control District for any applicable permits. <br />2. Contact Jeffrey Wong (468-0335) to arrange the following inspection. A 48-hour <br />notice is required: <br />a. The EHD must witness the functionality of the monitoring system to <br />ensure that monitoring has been restored to meet the requirements of <br />California Code of Regulations Title 23 and the Health & Safety <br />Code. All monitoring system sensors must be tested. <br />3. A current printout of the alarm history must be created and available for <br />EHD inspection before it is cleared by any part of this work such as a "cold <br />start". <br />4. Any changes shall be submitted to the San Joaquin County Environmental Health <br />Department (EHD) for approval prior to commencing work. All new addenda <br />supersedes the previous ones. <br />5. If concrete is broken over or near the existing underground storage tanks (UST), a <br />tank secondary containment test must be performed on the UST in accordance to <br />manufacturer's guidelines. Testing must be done after returning the facility to <br />operating condition. Results must be submitted to EHD within 7 days of <br />returning the facility to operating condition. <br />6. All components must be certified by an independent testing organization and <br />labeled as required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See <br />Page 1 of 2 <br />
