The component must bear the appropriate mark as required by the <br />independent testing organization. <br />7. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all <br />of the following: <br />a. One of several permissible valid, current contractor's licenses issued by <br />the Contractors State License Board (may be issued to the installation <br />company), and <br />b. All valid, current California UST certificates issued by the International <br />Code Council (ICC), that are applicable to the work being performed, and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced; and <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />8. An "Underground Storage Tank Certification of Installation/Modification" form <br />(Form C) shall be submitted by the owner or the owners' agent [pursuant to <br />(CCR) 2635(d)] before this upgrade will be considered complete by EHD. <br />9. Permit expires 180 days from date of approval.
