UST Tank Information forms for the [note which ones] are not current in CERS. -The tank forms incorrectly state that the site has a vent sump. -The vent piping and vapor recovery piping are marked as having secondary containment. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. -The monitoring plans for all four tanks states the monitoring panel is a TLS -350. The monitoring panel is a Simplicity. -The monitoring plan for the 87 tank includes notes regarding the 89 and 91 tanks. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a Class II violation. 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. A monitoring system certification, leak detector testing, and spill container testing were performed on 5/31/17. A test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). A copy of the test report must be submitted within 30 days of the test. Immediately provide this report to the EHD. This is a Class II violation. 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on May 31, 2017 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by June 30, 2017 . This information has not been received resulting in a non-compliant status for this facility. The outstanding violations include: -UST system is not operated to prevent unauthorized releases. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a repeat violation, Class II. 206 CCR 2712 Failure to comply with UST operating permit conditions. A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. This is a Class II violation.
