This is aviolation of California code of regulation(CCR)section 2712 <br /> (b) Immediately start recording all alarms and how they are resolved. The <br /> three instances listed are from records reflecting 300 days alarm reporting. <br /> With respect to the first and second of the instances referred to on 1/29/08 <br /> and 5/17/08,the O alarm records enclosed clearly show that no alarms <br /> automatically recorded on those dates. Therefore,the alarm records <br /> maintained by the facility are entirely accurate and were appropriately <br /> maintained by the facility's employees. No response action was taken <br /> because there was nothing to which they needed to respond. The third,on <br /> 9/4/07,was misrecorded on 9/5/07,together with the response action taken. <br /> Therefore,during the relevant 300 day period,one of the three noted <br /> instances was a clerical error,and the other two were recorded accurately <br /> and required no action. Flying J has reviewed with our employees the need <br /> for absolute accuracy in recording alarms and the need to correctly note on <br /> the form how the alarms were resolved. <br /> 2. 12. 89 and 91 leak detectors failed 3 gal/hr test at 10 psi. This is a violation of <br /> Health and Safety Code section 25291(b) The service technician changed the <br /> leak detectors(For 91 LD—FX2 V SM 5562, 89 LD—FMV SM 7915.) The leak <br />
