Any release from the primary containment that does not escape the secondary <br /> containment and was cleaned up within eight hours of being detected shall be recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary <br /> containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's monitoring <br /> records. Submit a copy of the recordable release statement to the EHD. <br /> This is a repeat violation, Class Il. <br /> 309 CCR 2636(f)(1) Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The T3 diesel STP 208 sensor failed to activate an audible and visual alarm when tested to ensure continuous <br /> monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or <br /> stop the flow of product at the dispenser when it detects a leak. The service technician placed the sensor upside <br /> down, and it went into alarm.The service technician then retested the sensor, it passed. <br /> This was corrected onsite. <br /> This is a repeat violation, Class ll. <br /> 313 HSC 25290.1,25290.2,25291 Failure to construct, operate,and maintain primary containment as product-tight. <br /> Oily staining was noted on the floor of the biodiesel shed, and pooling of oily liquid was noted inside the biodiesel shed <br /> sump, indicating a leak in the primary piping. All primary containment for the UST system must be tight. Immediately <br /> have a properly licensed,trained, and certified contractor repair or replace the failed component under permit and <br /> inspection of the EHD. <br /> This is a repeat violation, Class ll. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the T1,T2,T3,T4 STP sump on 5/3/17. If water could enter into the secondary containment <br /> by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a <br /> hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly.
