The facility failed to document <br />the L8: diesel 2 annular setup data warning alarm that occurred on 4/13/2018. These records shall be maintained <br />on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) <br />monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of <br />sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, <br />ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately <br />locate and maintain all maintenance and monitoring records for the last three years on site and submit a statement <br />on how the alarm was addressed. <br />This is a minor violation. <br />204 <br />CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br />An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br />accessible on site at all times. Immediately locate and retain a copy on site. <br />This is a repeat violation, Class II. <br />206 <br />CCR 2712 Failure to comply with UST operating permit conditions. <br />Work orders from Jones Covey were found on site for maintenance work that was conducted on 12/5/2017, <br />1/22/2018, and 4/7/2018, which indicated that leak detectors and sensor were replaced. No notification were made <br />to EHD for these like for like replacements and the conditions for the no permit required work policy was not <br />followed. On 1/23/2018, Isaac Garcia contacted EHD about information regarding leak detector replacement and <br />EHD policy was submitted to him. No further correspondence was made. Emergency repairs relating to UST system <br />monitoring and release detection may be performed by a properly licensed, trained, and certified contractor <br />pursuant to the EHD's policy and a permit obtained within one business day from the EHD. All other repairs must <br />have a permit submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit from the <br />EHD for the above mentioned repairs. A "failure to obtain a permit" penalty applies. Submit proof of correction to the <br />EHD. <br />This is a minor violation. <br />209 <br />CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to document all the alarms from the attached alarm history on the December 2017 <br />designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br />missing alarms include: three L29: Disp 19 Satellite fuel alarms for 11/21/2017. During the monthly inspection, the <br />designated operator shall review the alarm history for the previous month, check that each alarm was documented <br />and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report.
