Flag City Shell <br /> 6437 Banner St., Lodi <br /> 6/28/96-3 double walled (20,000 gallon 87 Unleaded 10,000 gallon, 91 Premium 10,000 <br /> gallon, & Diesel) Trusco UST's and double walled fiberglass pipes were installed. <br /> 6/2/97-Leak detector failed and was replaced on the Premium tank, witnessed by <br /> Michael Kith, RENS. <br /> 2/10/99-Operation changed from "Exxon Tiger" to "Flag City Shell", owner New West <br /> Petroleum. <br /> 9/5/01-Routine UST inspection revealed leak detector failed on Unleaded tank and was <br /> replaced under EHD permit onl0/15/01. <br /> 6/17/02-Routine UST inspection found no major violations, all monitoring certifications <br /> passed. <br /> 12/5/02-Secondary containment testing was done pursuant to SB 989, everything <br /> passed. <br /> 4/16/03-Monitoring certification testing done, all passed. <br /> 5/6/03-Routine UST inspection by Dennis Catanyag revealed a stick in the 10,000 <br /> gallon Diesel fill pipe overriding the overfill prevention flapper valve and preventing it <br /> from functioning. <br /> 7/18/03-Leak detector replacement application received and approved by EHD. <br /> 7/23/03-Leak detector replacement inspection by Dennis Catanyag revealed two more <br /> sticks found in 87 Unleaded fill pipe, but not in the Diesel or the Premium. <br /> 9/2/03-File referred to the District Attorney's office for manually overriding the overfill <br /> prevention system. <br /> 12/1/03-Spill containers tested pursuant to AB 2481, 87 Unleaded and Diesel spill <br /> containers failed. <br /> 2/19/04-Spill containers replaced under EHD permit. <br /> 5/13/04-Routine UST inspection, no major violations. <br /> 4/25/05-Routine UST inspection. Passed Monitoring Certification and spill containers <br /> passed. Discovered the monitoring panel was moved to a different location which <br /> required a "cold start" monitoring cert but no permit was obtained from EHD for this <br /> work. <br /> 11/29/05-Received and approved application for ELD (Enhanced Leak Detection) <br /> testing. <br /> 1 <br />� J <br />
