12/16/05-Secondary containment test (36 month requirement) was performed and spill <br /> containers were also tested. Secondary containment components passed but 87 <br /> Unleaded spill container failed. <br /> 12/20/05-Received application to replace spill container, approved on 12/21/05. <br /> 12/27/05-EHD witnessed the installation and testing of the 87-replacement spill <br /> container, it passed. <br /> 1/11/06-EHD Accounting office received information that ownership change has <br /> occurred and the new owner of record is: J. Gilbert Moore, New West Stations, Inc., <br /> DBA: Flag City Shell. <br /> 1/16/06-ELD testing performed, 87 Unleaded vapor bucket (not under EHD jurisdiction) <br /> failed the ELD test but all others passed. <br /> 3/3/06-Received and approved application to run new wiring to the monitoring panel. <br /> 4/10/06-Routine UST revealed monitoring panel read-out is showing multiple "Sensor <br /> out' alarms. When asked about this the manager said there was a problem with the <br /> "shielded cable" that Alpha Petroleum obtained a permit from this office to correct. <br /> 4/26/06-Lori Luces, REHS, called Alpha Petroleum to see when the work will be done <br /> and was told they would not be doing any work as the problem is with electrical wiring <br /> and not the shielded cable as they thought. <br /> 5/3/06-Lori Luces faxed a letter to the owner notifying them to begin work by 5/5/06 or <br /> the facility will be considered out of compliance. <br /> 2 <br />
