The <br />Response Plan dated 1/3/13 in CERS lists Jivtesh S. Gill and Stephen Boyd as Responsible Persons, which differs <br />from the names, Patrick McConnell and Damon Brown, listed as Responsible Parties in the 10/24/12 Response Plan. <br />The response plan on site at the facility must be current. Ensure a current copy is available for inspection at any <br />time. <br />The monitoring plan is not current and/or not approved by the EHD. The Mechanical Line Leak Detector (MLLD) <br />manufacturer for the midgrade (89) unleaded product is VAPORLESS and the MLLD Model is LD -2000. The <br />monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into <br />CERS, make the necessary changes, and submit for review by the EHD. <br />This is a repeat violation, Class Il. <br />109 <br />CCR 2637 Failed to test secondary containment. <br />Secondary containment testing was last performed on 12/10/12 and was completed on 2/10/16, two months past due. <br />Secondary containment testing is required once every 36 months. Take all necessary precautions to ensure testing is <br />performed in a timely manner. <br />This is a Class II violation. <br />207 <br />CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to check and note the correct test dates for the secondary containment test on the <br />12/30/15 and 2/24/16 designated operator monthly inspection checklist report. Both reports listed 12/7/15 as the date <br />when the secondary containment testing was completed. Although hydrostatic testing of STP containment for 87, 89, <br />and 91 products was completed on 12/7/15 as part of the secondary containment testing, the remainder of secondary <br />containment testing that included hydrostatic testing of the secondary piping for 87, 89, and 91 products and UDCs # <br />1/2, 3/4, 5/6, 7/8,9/10 and 11/12 was not performed until 2/10/16. During the monthly inspection, the designated <br />operator shall check that all required testing and maintenance for the UST system have been completed and <br />document the dates they were done. Ensure that designated operators performing monthly inspections at this facility <br />are including all of the required information on the reports. <br />This is a minor violation. <br />308 <br />CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br />The 208 sensor in the 91 product fill sump failed to activate an audible and visual alarm when tested to ensure <br />continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and <br />visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor was replaced and retested <br />during the inspection. A "UST Retrofit Verification with Inspector Already Onsite" form has been completed and <br />provided to the operator and contractor. <br />This was corrected on site. <br />This is a minor violation. <br />Page 4 of 6 <br />
