An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be performed. Include the locations of the line leak detectors. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. Secondary containment testing was performed on May 12, 2016 and a test report was submitted on August 22, 2016, greater than two months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 205 CCR 2712(i) Failure to have an approved UST response plan available onsite. An approved copy of the Response Plan was not available for inspection. A copy of the Response Plan shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the Secondary Containment testing on the June 29, 2016 through April 26, 2017 designated operator monthly inspection reports. The designated operator inadvertently entered 5/23/16 on all of the designated operator monthly inspection reports instead of the correct date, 5/12/16. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation.
