A licensed contractor must be named with the following information provided: <br /> a. EHD contractor's and subcontractor's questionnaire (ignore if already on file) <br /> b. Certificate of worker's compensation insurance <br /> c. Documentation of a Hazardous Substance Removal Certification <br /> 2. Site Health & Safety Plan <br /> 3. Contractor's license number for the contractor performing decontamination(Shaw <br /> Environmental) <br /> 4. Name of sampling firm <br /> 5. Question#1 lb on the permit application"What will be the final destination of the <br /> excavated stockpile". A destination, i.e., TSDF, Landfill, etc. must be named in the event <br /> that the stockpile is determined to be contaminated. <br /> 6. On the Backfill Excavation Certificate the method indicated was to use crushed rock <br /> but on#10—pea gravel will be used as backfill. Crushed rock will not be allowed to be <br /> in contact with the tank or flex piping. <br /> 7.
