Diesel fuel was found at the Diesel STP sump. Per technician, he removed approximately 2 cups of <br />fuel with absorbent pads. Technician used a clean white absorbent pad to wipe along the opening of <br />secondary fiberglass piping, primary fiberglass piping, flex piping, turbine and ATG risers, sump wall, <br />manway & bolts and found no areas of possible seepage. All primary containment must be product tight. <br />Correct by monitoring the diesel STP sump for areas of possible seepage on a regular basis. Have a <br />properly licensed, trained, and certified contractor replace or repair the failed piping under permit and <br />inspection of the EHD. <br />311. The premium unleaded mechanical line leak detector failed to detect a leak when tested. All line <br />leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician <br />adjusted the leak detector once, retested it, and it passed. <br />113. A new designated operator Douglas L Nunn had started the monthly inspection reports in July 2013 <br />to present and notification was not provided to the EHD within 30 days of the change. Any changes <br />shall be reported to the EHD with 30 days. Immediately, submit notification to the EHD identifying all the <br />designated operators for this facility. Alternatively, log into CERS, upload the required information, and <br />submit for review by the EHD. <br />- Corrected on site. Copy of the Owner Statement of DO was found with inspector's name & fax # but no <br />copy was received by EHD. <br />206. Failed to include hiring date of employees trained by DO. DO training records were missing <br />employee's hiring date. List shall include dates of training and also hiring dates for all facility's <br />employees hired on or after July 1, 2005. <br />- Corrected on site.
