Lafayette St., Stockton <br /> The San Joaquin County Environmental Health Department(EHD) has completed the review of <br /> the revised application for a permit to remove the following underground storage tank (UST) <br /> systems, including piping and associated appurtenances, at the above noted address: <br /> Tank#39-0520435: 8,500 gallon unknown <br /> Tank#39-0520436: 8,500 gallon unknown <br /> The proposed amendments (submitted December 22 & 26, 2011) include removal of additional <br /> UST piping discovered during the removal, including under a building on site. <br /> The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Stacy Rivera at(209) 468-3440 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a. Removal of tanks, piping, and associated appurtenances (Tank removal <br /> witnessed 12/13/11) <br /> b. Witness soil and/or water sampling (Tank pit, partial piping trench, and stockpile <br /> sampling witnessed 12/13/11, 12/16/11, and 12/19/11) <br /> c. Verification of Tank ID numbers marked on the USTs (Witnessed 12/13/11) <br /> 4. If the removed backfill material and soil is found to be contaminated it must be disposed <br /> of at an approved disposal location. Treatment of soil onsite can only be performed <br /> under a hazardous waste treatment permit. A work plan and fees must be submitted to <br /> the EHD Site Mitigation Unit and the work plan must be approved before initiating any <br /> assessment or remediation activities. <br />
