Peters, Jr. —Sr. Director- Environmental, Health & Safety <br /> 4.18.3 Location Managers and DEAs shall choose only an Approved Waste Tire Hauler <br /> when selecting a company to perform any on-site tire maintenance on vehicles <br /> and/or lift equipment which will result in the removal of 10 or more rubber tires from <br /> the Location and subsequent transport for recycling at any one time. (Refer to <br /> Appendix E for the link to the list of Approved Waste Tire Haulers). <br /> 4.18.4 Location Managers and DEAs are prohibited from allowing Approved Waste Tire <br /> Haulers to take 10 or more tires at any one time from the HD Supply Location when <br /> the Approved Waste Tire Hauler does not present to the Location Manager a <br /> Comprehensive Trip Log at time of service. In such instances, please contact the <br /> EHS Team. <br /> 4.18.5 Location Managers and DEAs shall review the Waste Tire Hauler's Comprehensive <br /> Trip Log (CTL)for completeness and accuracy prior to initialing the CTL. <br /> 4.18.6 Location Managers shall retain signed copies of the CTL at the Location for three (3) <br /> years. Such documents shall be subject to periodic audit. <br /> 4.19 Episodic Generation <br /> 4.19.1 Only certain states have adopted the Generator Improvement Federal Rules, which <br /> allows for episodic generation. Only states listed in Appendix G can use episodic <br /> generation. <br /> 4.19.2 If a location is a VSQG or SQG and have accumulated or will be accumulating a <br /> larger than normal amount of RCRA hazardous waste, contact the Enterprise EHS <br /> department as soon as possible so that it can be determined whether episodic <br /> generation should be used for the event. <br /> 4.19.3 The DEA shall complete the episodic generation request form located in Appendix H <br /> and send the completed form to gsc.ehs.info@hdsupply.com <br /> 4.19.4 The Enterprise EHS department must notify the state in order to take advantage of <br /> episodic generation 30 days prior to a planned event or within 72 hours of an <br /> unplanned event. <br /> 4.19.5 The Enterprise EHS department will provide the branch with additional details for <br /> managing the event in accordance with the regulations. <br /> 4.19.6 VSQGs and SQGs are limited to one episodic event per calendar year unless a <br /> petition is granted from the Regional EPA administration allowing for a second <br /> episodic event in the same calendar year. <br /> 5.0 TRAINING <br /> 5.1 All Business Leadership, Business EHS Teams, Affected Associates, DEAs and DECs shall <br /> be familiar with the requirements of this Standard and shall receive all required training. <br /> 5.2 Required training for Affected Associates includes the following: <br /> Any printed copy of this document is an uncontrolled document.
