The only training found during the <br /> inspection is the ammonia evacuation training. The business plan shall include provisions for ensuring that <br /> appropriate personnel receive initial and annual refresher training.All employees shall be trained in safety <br /> procedures in the event of a release or threatened release of a hazardous material, including, but not limited to, <br /> familiarity with the following plans and procedures: <br /> (A)Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property,or the environment. <br /> (C)Evacuation plans and procedures, including immediate notice, for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Onsite to conduct a Hazardous Materials Business Plan ( HMBP) inspection. There were four violations during <br /> the inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a <br /> statement documenting the corrective actions that have been or will be taken for each violation, and any <br /> supporting paperwork, by 12/26/2019. <br /> Notes: Approximately 1000 gallons of Perasan (Approximately 2,150 pounds of Peractic Acid)found onsite at <br /> the time of the inspection without a CaIARP program. The Threshold reportable quantity for Peracetic Acid is <br /> 500 pounds. Evaluate the process of storing the chemical and update SJC EHD of adding the chemical to the <br /> CaIARP Program. <br /> We were not allowed to take photos during the inspection. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
